National Disability Insurance Scheme (NDIS)
Care Pathways Australia is a registered NDIS provider, offering support for self-managed, plan-managed, and NDIA-managed participants.
We provide services under the following categories:

• Assessments (Functional, Developmental, Cognitive, or Diagnostic)
• NDIS-funded Group Programs (Programs of Support model)
How it works:
1. Submit your referral or service request form.
2. Our intake team confirms your NDIS plan details and available budget category.
3. A service agreement is prepared for review and signing.
4. Sessions commence as per your agreed schedule (home visits, telehealth, or ELC/School/Kinder).

Care Pathways Australia NDIS Audited and Approved Registration Groups
1. Therapeutic Supports (Registration Group 0128)
Purpose: To help participants improve functional capacity, independence, and participation in everyday life.
Professionals: Occupational Therapists, Physiotherapists, Speech Pathologists, Psychologists
Services Provided:
• Occupational Therapy for daily living, sensory regulation, fine/gross motor skills, and home safety
• Speech Pathology for communication, language, literacy, feeding, and swallowing
• Physiotherapy for mobility, balance, rehabilitation, and pain management
• Psychology for emotional regulation, mental health, and behaviour support
• Functional capacity and goal-based assessments
• Therapy programs for children, adolescents, and adults
• Home, school, and community visits
• Telehealth therapy sessions Australia-wide
2. Early Intervention Supports for Early Childhood (Registration Group 0118)
Purpose: To support children aged 0–6 years with developmental delay or disability to reach their full potential through early intervention.
Professionals: Early Childhood Specialists, OTs, Speech Pathologists, Physios, Psychologists
Services Provided:
• Play-based therapy and parent coaching
• Early communication, language, and social skill development
• Sensory processing and self-regulation strategies
• Fine and gross motor skill programs
• Feeding and daily routine support
• Collaboration with educators and childcare staff
• Family capacity building and goal setting
• Group programs and school readiness
3. Innovative Community Participation (Registration Group 0116)
Purpose: To help participants build confidence, social connection, and independence through community-based skill development.
Services Provided:
• Social and life skills development sessions
• Community access support (shopping, cooking, budgeting, transport training)
• Emotional and behavioural coaching in real-world settings
• Confidence-building activities (e.g. gym, group outings, workshops)
• Transition to work, study, or volunteering support
• Supported community participation for teens and adults
4. Communication and Information Equipment (Registration Group 0103)
Purpose: To assist participants who need assistive technology for communication, access, or daily function.
Services Provided:
• AAC (Augmentative and Alternative Communication) assessments
• Prescription and trial of communication devices or software (e.g. iPads, speech-generating devices)
• Assistive technology (AT) recommendations and reports for NDIS approval
• Training for families, schools, and carers on equipment use
• Ongoing review and re-assessment of device suitability
5. Home Modification Design and Construction (Registration Group 0124)
Purpose: To help participants remain safe and independent at home through environmental modifications.
Professionals: Occupational Therapists (in collaboration with builders and certifiers)
Services Provided:
• Home safety and environmental assessments
• Design and recommendation of modifications (e.g. bathroom, ramps, access pathways)
• Functional reports and quotes for NDIS funding submission
• Coordination with licensed builders and project management
• Post-modification follow-up and functional review
Medicare
Medicare rebates are available for eligible clients with a GP referral.
You may be able to access allied health sessions under a Chronic Disease Management Plan (CDMP) or Enhanced Primary Care Plan (EPC).
What this means:
• Up to five (5) sessions per calendar year may be rebated.
• Available for occupational therapy, speech pathology, physiotherapy, or psychology.
• You’ll need a referral from your GP or specialist to access the rebate.
• Gap fees may apply depending on your funding arrangement.
Private Health Insurance
Many private health funds offer rebates for allied health services, depending on your level of cover.
You can claim for:
• Occupational Therapy
• Speech Pathology
• Physiotherapy
• Psychology
Claims can often be processed on the spot or submitted through your health fund’s app or portal.
We recommend checking with your provider to confirm your eligible rebate amount per session.
Aged Care Programs
For older adults (65+), our services can be funded through:
• Support at Home (commencing November 2025)
We work directly with aged care providers to coordinate allied health services such as physiotherapy, occupational therapy, speech pathology, and psychology to support mobility, safety, and wellbeing at home.
Our team can help liaise with your aged care provider to ensure therapy is included in your care plan.
