At Care Pathways Australia, we’re passionate about helping older Australians live well at home.
Our in-home allied-health services are designed to support ageing with dignity - promoting independence, mobility, safety, and emotional wellbeing.

We bring therapy to where it matters most: your home and your community.
Whether recovering from illness, managing a chronic condition, or simply maintaining strength and confidence, our goal is to help you continue doing the things you love.

Our in-home services

We provide therapy and assessment across all major allied-health disciplines:
Occupational Therapy — home safety assessments, mobility aids, equipment prescription, falls-prevention strategies, and functional capacity assessments.
Physiotherapy — strength, balance, pain management, and movement training to improve confidence and reduce falls risk.
Speech Pathology — swallowing assessments, speech and language therapy, cognitive-communication support, and meal-time safety programs.
Psychology — emotional health, coping with change, adjustment to ageing, grief support, and mental wellbeing.

Additional support

• Cognitive and memory screening
• Home modification recommendations
• Reablement and restorative care programs
• Gentle exercise or hydrotherapy sessions
• Family and carer coaching

• Easy-to-use referral forms and intake guides

Our clinicians provide consistent communication so families feel informed, supported, and empowered every step of the way.

Our approach

Our care model is grounded in person-centred practice — we take the time to understand what matters to you.
We assess your strengths, home environment, and goals to design a tailored therapy plan that fits your lifestyle.
