Getting onto the National Disability Insurance Scheme (NDIS) can sometimes feel overwhelming – there are forms to fill out, assessments to complete, and a lot of new information to take on board.
At Care Pathways, we understand how challenging this process can be for families, both practically and emotionally. It can feel like a demanding task especially when you’re already focused on supporting your child’s everyday needs.That’s why we’re here – to make the journey easier, clearer and more supportive from the very first step, right to the end. Whether you’re just starting to explore NDIS eligibility or already have a plan in place, our team is ready to walk alongside you throughout this process.
What is the NDIS and how do you apply?
The National Disability Insurance Scheme (NDIS) is a government-funded program that provides support to Australians with permanent and significant disabilities. It helps people access the services, therapies, equipment, and community connections they need to live more independently and achieve their goals.
The NDIS can also fund services that give families and carers a break, such as community participation or short-term accommodation.
Steps to apply for NDIS:
Connect with a local partner
If your child is under 9 years old, contact an early childhood partner. ECA (Early Childhood Approach) fund the same things as the NDIS. ECA was developed to ensure children can get the support as early as possible to reduce the impact of disability later in life.
If the child is 9 years old or above, reach out to a local area coordinator (LAC).
How do I find my local area coordinator or early childhood partner?
Visit the office location page on the NDIS website to search for offices in your area or call them on 1800 800 110.
These partners help you understand if you’re eligible, gather your documents and even submit your document.
Understand the eligibility criteria
To qualify you must:
Be under 65 years old
Be an Australian citizen, permanent resident, or Protected Special Category Visa holder
Gather the right evidence
Be prepared to provide:
Proof of identity, age and residence
Medical evidence from a health professional (e.g. GP, pediatrician) who has treated your child for at least 6 months
Reports or assessments showing how the disability affects daily life
Submit your application
Your partner can help you submit your Access Request Form.You’ll be assigned an NDIS contact to support you throughout each stage of – applying for the NDIS, creating your NDIS plan and using your NDIS plan.
What happens next?
The NDIS will respond within 21 days and if you’re eligible you’ll receive a Notice of Impairments and a letter confirming your participant statement.You’ll then work with your contact to create your first NDIS plan. Your NDIS plan will be based off discussions in your planning session and will include five main sections: information about you, family and friends, services and community groups, your goals and funded supports.

Getting started with Care Pathways
When you begin services with us, you’ll be supported by a team of professionals working together to provide the best care for your child and family.
Here's a brief overview of what you can expect at your first appointment:
Comprehensive intake interview
We’ll start with a detailed conversation to understand your child’s strengths, challenges, and developmental history.
Service agreement
This outlines the supports and services your provider will deliver to you under the National Disability Insurance Scheme (NDIS). This includes services, costs, responsibilities of you and your provider, and how to make changes if needed—ensuring you clarity and security.
Consent documentation
You’ll complete a consent form that explains what you’re agreeing to, for how long, and with which services.
Two of our practitioners will attend your first appointment so that both can begin to understand your child’s history and have the opportunity to see where your child is up to. This also provides an excellent opportunity for you to meet two staff, from your local team, who will be involved in the care of your child.
This means that there will be a charge to your child’s NDIS package for both staff and their associated travel costs.
